Registration has closed for the 2011 season. Registration for the 2012 season will begin in April 2012. General registration information is included below.

Required Registration Materials

1. Complete and sign the CYSA membership form. Please add your child's short and shirt size on the application. If downloading the Membership form from the CYSA website, please print TWO copies and sign both original membership forms in blue ink .
2. Parent Sideline Ethics Agreement Policy (Microsoft Word document)
3. Special Request Form (if applicable)
4. "Play Up" Waiver Request Form (applicable if player wishes to play in an older age group)
5. Copy of birth certificate - For all new players to EGYSL or the Monterey Trail Soccer Club
6. If playing U9-U14, a small photo (1" x 1") - please write name of child on back of picture

Incomplete registrations will not be processed until ALL information required is received and may be returned to the parent/guardian.

Age Requirements for Players

Players must be at least 5 years old or entering kindergarten in order to be eligible to play in the EGYSL. If you're not sure which age level your child will be eligible for this coming season, please use the Age Calculator.

For children 4 years old and not in kindergarten, EGYSL is offering a U5 Academy. Please continue to monitor the EGYSL website for details.

Registration Fees from April 1, 2011 to May 31, 2011

$130 for the 1st and 2nd child
$100 for 3rd child
$80 for 4th child
$50 for 5th child
(Children must be in the same immediate family)

Registration Form for Coaches

Are you interested in coaching a Monterey Trail soccer team for the 2011 season? The first step is to fill out a CYSA Registration and Risk Management Disclosure Form. After completing this online form, print two copies and submit one of them to the Monterey Trail Soccer Club. You can submit your form by mail, or you can submit it in-person at any of the walk-in registration dates.
